LIABILITY WAIVER & MEDICAL RELEASE Soccer and the striker position are physical activities that have certain inherent risks that could result in physical harm, and, in extreme cases, death. I understand the risks involved and I hereby release the Chris Wehan Soccer Academy Academy and it's employees from any and all liability for any physical injuries or illness that may occur to the above named athlete while attending training. I have no knowledge of any physical impairment that would be affected by the above named athlete's participation in training activities. Furthermore, I hereby authorize the staff of Chris Wehan Soccer Academy to act on my behalf according to their best judgment in any emergency situation requiring medical attention.

Registration will be handled on a first come first serve space. Spaces are limited, just by filling out registration doesnt guarantee a spot in Strikers Academy or Mentor Program.
Payments can be made by cash, venmo or check made out to Chris Wehan Soccer Academy.

Code of Conduct

Athletes must respect fellow athletes, coaches, and Bosque High School.
Bring positive energy to each session.
Communicate in a timely manner for missed sessions.
Parents are always welcome to watch sessions from the outside.
No parent coaching during sessions.
Athletes who display harm to other athletes, disrespect to anyone, or are under the influence of drugs or alcohol will be suspended from the academy immediately. Suspension will be at my own discretion and will be communicated to both athlete and parents. Excessive absence or lack of communication may result in suspension as well.
